Adult Permission Form for Paint-A-Thon

VOICES ONE-TIME PERMISSION FORM                             

     If you wish, instead of filling out a permission form each time an event requires one, fill out one of our ONE-TIME forms to be kept on file. 

This form is applicable to events and activities covered by the St. Thomas of Villanova High School Youth Group, Voices.   It will be up to the parents to update any items on the form, such as allergies, medications, etc.  There may be specific events where this form may not cover a teen, like if the Archdiocese of Chicago is producing the event.  In cases like that, a separate form will be provided.
